The words can be confused because a 'Fehlentscheidung' is also a 'Fehler'. But not every 'Fehler' is a 'Fehlentscheidung'. For example, if someone writes a word incorrectly, that is a 'Fehler', not a 'Fehlentscheidung'. If someone chooses between options and the choice has bad consequences, 'Fehlentscheidung' fits better.
