'die Verfassung' and 'die Staatsform' both belong to the political structure of a state. However, 'die Verfassung' is the fundamental legal document or set of rules of a state. It contains, for example, rights, duties, and the organization of state authority. 'die Staatsform' is only one part of that: it describes how a state is basically organized, for example as a republic or a monarchy. The words can be confused because a constitution often also defines the form of government. If you mean the entire fundamental law of a state, 'Verfassung' fits; if you only mean the type of state, 'Staatsform' fits.
