'der Extremist' mainly has extreme views and often rejects democratic rules; however, he does not necessarily have to carry out an attack. 'der Terrorist' goes further: he carries out terror, meaning violence or attacks, in order to spread fear and achieve goals. The words can be confused because terrorists are usually also extremists. But not every extremist is automatically a terrorist. When it is about ideas and radical positions, 'Extremist' fits. When it is about terror and attacks, 'Terrorist' fits.
