'Der Gag' in German only means 'joke' or 'punchline'. The English verb 'to gag' (to choke, to muzzle) has a completely different meaning that the German noun does not have.

'die Idee' and 'der Gag' can both mean an idea. 'Idee' is neutral and can fit any topic: work, everyday life, art, or planning. 'Gag' is more specific. It is either a funny idea meant to make people laugh, or a surprising gimmick that gets attention. The words can be confused because a 'Gag' is often also an idea. But not every 'Idee' is a 'Gag'. If something is mainly useful or serious, you say 'Idee'. If it is meant to be funny, surprising, or eye-catching, 'Gag' fits better.
