Careful: 'Realisieren' has a double meaning. Like the English 'to realize', it can mean 'begreifen/verstehen' (to grasp/understand). Very often in German, however, it also means 'verwirklichen/umsetzen' (to make a reality/implement), which the English word does not mean in everyday use.
Sie will ihren Traum von einer Weltreise endlich realisieren.
She finally wants to realize her dream of traveling around the world.
Like all verbs ending in '-ieren', 'realisieren' forms its past participle without the prefix 'ge-'. The correct perfect tense form is 'hat realisiert' (has realized/implemented).
Ich habe erst jetzt das ganze Ausmaß des Problems realisiert.
Only now have I realized the full extent of the problem.
If a 'dass' clause follows, 'realisieren' usually means 'begreifen' (to grasp/realize). If a noun like 'Plan' (plan), 'Projekt' (project), or 'Traum' (dream) follows, it means 'verwirklichen' (to realize/make a reality).

'verstehen' and 'realisieren' can both mean that you grasp something. 'verstehen' is broader and fits explanations, languages, texts, or problems. 'realisieren' is used when someone becomes aware of something, often suddenly or after a moment. It is often about a truth or connection that was not clear before. Learners may confuse the words because both have to do with thinking. As a guide: you understand an explanation, but you suddenly realize that you are late.
Im Zug realisiert sie, dass sie den Termin vergessen hat.
