'Aufmerksamkeit' is more general and often smaller: it can be a flower, a card, or a kind gesture. 'Geburtstagsgeschenk' is more specific and names the occasion: it is given for a birthday. The words can be confused because a birthday present can also be an 'Aufmerksamkeit' (small gesture / thoughtful gift). But not every 'Aufmerksamkeit' is a birthday present. If the birthday is important, it is better to say 'Geburtstagsgeschenk'.
