The difference is mainly gender and form: 'der Nachfolger' is the masculine form and can also be meant generally in some texts. 'die Nachfolgerin' clearly says that a woman is taking over. The words can be confused when you are speaking quickly about a new person in a position. If the person is a woman and you want to be precise, 'die Nachfolgerin' is more natural.
