'der Kai' and 'der Pier' are both structures by the water where ships can dock. 'der Kai' is a reinforced waterfront wall directly at a harbor or riverbank and is often used for work in the harbor. 'der Pier' usually extends out into the water and can also be a promenade for tourists. The words are easy to confuse because both occur in harbor settings. As a simple guide: 'Kai' is more likely to be along the shore, while 'Pier' is more likely to lead out into the water.
