Register and origin
'Follower' is a modern loanword from English. It is used especially in the digital world and in the context of social media, where it has become the standard term.
EXAMPLE
Dieser Account hat nur wenige Posts, aber sehr viele Follower.
This account has only a few posts, but a lot of followers.
Context clue
The meaning in the social media context is by far the most common. When 'Kanälen' (channels), 'Accounts' (accounts), or 'Posts' (posts) are mentioned, this meaning is always intended. The more general meaning 'Anhänger' (supporter/adherent) is less common.

You do not have to know this person personally. Even as a supporter of an idea, 'Follower' sounds more like an adherent than a personal friend. The risk of confusion arises especially online. A follower is not automatically a friend.
