'Intensität' refers especially to the strength or force of something, for example pain, light, noise, or a feeling. The words can be confused because both describe a measure. When you are asking generally how far or how strongly something is developed or present, 'Grad' often fits. When the noticeable strength is the main focus, 'Intensität' sounds more precise.
