Word composition
The word consists of 'lesen' (to read) and 'Zeichen' (sign/mark). So it is literally a 'mark for reading' that shows where you want to continue reading.
EXAMPLE
Ohne Lesezeichen vergesse ich immer, auf welcher Seite ich war.
Without a bookmark, I always forget which page I was on.
Context clue
You can tell from the context whether a physical or digital bookmark is meant. Words like 'Buch' (book) or 'Seite' (page) point to the physical meaning, while 'Browser' (browser), 'Webseite' (webpage), or 'speichern' (to save) refer to the digital meaning.

When referring to a physical bookmark, verbs such as '(ins Buch) legen' (to put/place in the book), 'benutzen' (to use), or 'verwenden' (to use) are used.

With digital bookmarks in a browser, other verbs are common. You can 'speichern' (save), 'anlegen' (create), 'hinzufügen' (add), or 'löschen' (delete) a bookmark.
