The word consists of 'un-' and 'entschlossen' (determined/decided). 'Entschlossen' is the past participle of the verb 'sich entschließen' (to decide). So 'unentschlossen' literally means that one has not yet made a decision.

'Unentschlossen sein' (to be undecided) is very often followed by a subordinate clause with 'ob' (whether), 'was' (what), 'welche' (which), or 'wo' (where), which describes the different options.

'unklar' and 'unentschlossen' can both have to do with inner uncertainty. They are especially confusable in sentences about people. 'sich über etwas unklar sein' means that you yourself do not yet know exactly what you think, want, or should do. 'unentschlossen' describes a person who is wavering between options and does not make a decision. 'unklar' can also be used for things and situations, while 'unentschlossen' is almost always used for people or their behavior. When it is directly about a decision between options, 'unentschlossen' is more precise.
