'Entscheidung' is derived from the verb 'entscheiden' (to decide). The ending '-ung' is a typical way of forming nouns from verbs in German. These nouns are always feminine ('die').
Ich muss mich schnell entscheiden.

The most important and most common verb combination is 'eine Entscheidung treffen' (to make a decision). Learners often incorrectly use '*eine Entscheidung machen'.

'die Wahl' and 'die Entscheidung' are closely related because both involve being faced with several possibilities. 'die Wahl' tends to emphasize the available options or the process of choosing. 'die Entscheidung' tends to emphasize the result: you have made up your mind and act accordingly. So you can say: first you have 'die Wahl', then you make 'eine Entscheidung' (a decision). The words can be confused because both occur when choosing. If several options are important, use 'Wahl' rather than 'Entscheidung'; if the fixed result is important, use 'Entscheidung' rather than 'Wahl'.
