'offiziell' and 'institutionell' can both have to do with authorities, organizations, or fixed rules. Use 'offiziell' when something has been confirmed, permitted, or done in an official capacity by the responsible body. 'institutionell' describes more that something belongs to an institution or is firmly embedded in an organization or society. The words can be confused because institutions often also make official decisions. In practice: a statement can be 'offiziell' if it has been confirmed; a problem can be 'institutionell' if it lies in the structures of an organization.
